I hereby:
- Authorize Street-wize to make the specified modifications, repairs, or restorations listed on my Preliminary Work Order/Estimate form.
- Grant Street-wize permission to operate my vehicle herein described on city streets, state highways, or elsewhere for inspection, testing, or delivery.
- Grant Street-wize permission to photograph my vehicle for use in Street-wize marketing and promotional materials including the Street-wize.com website.
I further acknowledge that Street-wize:
- Will not be held responsible for any loss or damage to my vehicle or personal belongings left in my vehicle in case of fire, theft, accident, or any other event beyond Street-wize’s reasonable control.
- All parts identified on the work order must be paid in full before work can begin.
- Is responsible for the disposal of unused, removed, or old parts removed from my vehicle unless otherwise instructed by me on the Street-wize Client Information Form.
Payment terms and methods
The total amount of the payment for the custom automotive work shall be paid in full or when agreed to by Street-wize and Client in installments as follows:
- Iterative payments to cover parts and labor are required within 7 calendar days of a Street-wize invoice date for all work orders unless otherwise recorded in the agreed-upon terms section of this agreement.
- Payment in full is due upon release of my vehicle to include any additional change orders, supplemental damage charges and research fees.
- Payments can be made in cash or debit/credit card.
- An additional fee may be applied to payments to cover fees for processing transactions through debit or credit.
